ONLINE FILLABLE FORM INSTRUCTIONS

Urgent Care of Hanford has made it easier for you to fill out new patient forms online using
the free Adobe Acrobat Reader. Below are some tips on using this option.

2. Some fields will auto-fill on all pages. Enter the date or date of birth once and it fills
throughout the document.
3. Use the tab button/key to navigate from one field to the next, or click on the desired area.
4. Once you are finished (f you are using a tablet or smart phone) you can sign them prior to printing.
Online fillable new Patient Forms is another way we try to make each patient’s visit a positive experience. However, if you prefer to print a blank form, and bring it to the office personally, select the file from the appropriate area below. You can then use the available print feature at the upper-right of your screen to do so.

If you would like to complete your form online, you may do so, then print it and bring the completed item with you. Just select the appropriate item from below. If you prefer to complete the form by hand, then you may print a blank form by simply opening the appropriate document from below and using the print icon toward the top-right:
